Create Waiver

PARENTAL CONSENT: I, _______________, as the parent or legal guardian of the minor participant, hereby grant permission for my child to participate in _______________ organized by _______________ on 2026-03-17. I confirm that I have the legal authority to provide this consent and to sign this waiver on behalf of the minor.

MINOR SUPERVISION: I understand that while _______________ will provide reasonable supervision during _______________, I acknowledge that constant one-on-one supervision may not be possible. I accept responsibility for informing _______________ of any special needs, behavioral concerns, or other relevant information about my child.

MEDICAL EMERGENCY AUTHORIZATION: In the event of a medical emergency involving my child during _______________, I authorize _______________ and its staff to seek and obtain emergency medical treatment, including but not limited to first aid, CPR, ambulance transport, and hospital care. I agree to be responsible for all medical costs incurred.

PHOTO PERMISSION FOR MINOR: I grant _______________ permission to photograph and/or video record my child during _______________ for use in promotional materials, social media, and other publications. I understand that my child will not be individually identified by name without my additional written consent.

BEHAVIORAL EXPECTATIONS: I agree to ensure that my child understands and follows all safety rules, instructions from staff, and behavioral expectations during _______________. I acknowledge that _______________ reserves the right to dismiss my child from the activity if their behavior poses a risk to themselves or others.

PICK-UP AUTHORIZATION: I will ensure that an authorized adult picks up my child at the designated time and location following _______________. I agree to provide _______________ with a list of authorized pick-up persons and to present valid identification upon pick-up. _______________ will not release the minor to any unauthorized individual.
