EQUIPMENT CONDITION ACCEPTANCE: I, _______________, acknowledge that I have inspected the equipment provided by _______________ for _______________ on 2026-03-17 and confirm that it is in acceptable and working condition. I agree that by accepting the equipment, I assume all responsibility for its condition during the rental period.

DAMAGE LIABILITY: I accept full financial liability for any damage to, loss of, or destruction of the rented equipment during the rental period, whether caused by accident, misuse, negligence, or any other reason, excluding normal wear and tear. I agree to pay the full repair or replacement cost as determined by _______________.

PROPER USAGE AGREEMENT: I agree to use the equipment only for its intended purpose and in accordance with all instructions, guidelines, and safety precautions provided by _______________. I will not modify, alter, disassemble, or attempt to repair the equipment without prior written authorization from _______________.

RETURN CONDITION: I agree to return all rented equipment to _______________ in the same condition as received, allowing for normal wear and tear, by the agreed-upon return date and time. Late returns may be subject to additional rental charges as specified by _______________.

REPLACEMENT COST: In the event that the rented equipment is lost, stolen, or damaged beyond repair, I agree to pay the full replacement cost as determined by _______________. I understand that the replacement cost may include the current market value of the equipment plus any applicable administrative or processing fees.

INSURANCE DISCLAIMER: I understand that _______________ does not provide insurance coverage for the rented equipment or for any injuries or damages that may occur during its use.
