Create Waiver

ASSUMPTION OF RISK: I, _______________, acknowledge that participation in _______________ organized by _______________ on 2026-03-17 involves inherent risks, including but not limited to physical injury, property damage, or other harm. I voluntarily assume all such risks, both known and unknown, even if arising from the negligence of _______________ or others.

RELEASE OF LIABILITY: I hereby release, discharge, and hold harmless _______________, its officers, employees, agents, and representatives from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury that may be sustained by me during or as a result of my participation in _______________.

INDEMNIFICATION: I agree to indemnify, defend, and hold harmless _______________ from any and all claims, damages, losses, and expenses, including attorney fees, arising from my participation in _______________ or my breach of any representation made in this waiver.

MEDICAL AUTHORIZATION: In the event of an emergency, I authorize _______________ to obtain or provide emergency medical treatment on my behalf. I understand that _______________ does not assume responsibility for any medical expenses incurred.
